Admission to the Foundation
The admission to the Foundation takes place, with the agreement of the patient and of his/her family, on aproposal of the nephrologists. This therapeutic choice takes into account the medical state, the family, professional and social environment of the patient. This option cannot be started without the patient's agreement to have dialysis.
This method of treatment needs a solid partnership between the patient and his/her family, of his/her medical pole and the board department of the Fondation Transplantation.
The patient's registration file for the National Health insurance is composed of:
- his/her information form,
- his/her agreement to practice dialysis,
- a preliminary request for reimbursement by the National Health Insurance,
- a copy of his/her Health Insurance Card.
